Growing packages in Rockhampton and Yeppoon

MANY of CentacareCQ‘s National Disability Insurance Scheme (NDIS) participants have now received their second NDIS Plan.
CentacareCQ is busily renewing Service Agreements with each and every one of them.
It is exciting to see most client’s Plan’s have remained the same or, even better, has increased.
It is also amazing to see the difference this support is making to the participant’s lives; ensuring they are achieving their goals in life.
At present, CentacareCQ is in the process of recruiting another NDIS Plan Facilitator for Rockhampton as Melissa moved on in January.
CentacareCQ Yeppoon case manager Don Wearne is finding his feet and is very enthusiastic about helping all clients on the Capricorn Coast have their needs met.
Don is able to help both clients over 65 years and clients who have NDIS funding.
Home Care Package clients are being supported to remain in their homes for longer in many different ways- from frequent visits a day, to supportive equipment to assist their independence (eg: grab tools, adaptive cutlery and hospital beds).
The flexibility of a Home Care Package means it is very individual and ideal for meeting each client’s needs.
Client Information Sessions are held every 3rd Wednesday of the month at 9am.
These sessions explain how to get the best from your funding and what to do if your funding is not meeting your needs.
